
Kick Starting Integrated Primary and Community Health Systems in Taranaki
Abstract
Using Community based adult physical health services as a case study within Taranaki a provincial region of New Zealand this presentation will outline how a collaborative approach between Primary, Community and Hospital Services has developed new models of care.
This project aimed to
- Design a detailed model of integrated care for primary care and community health services
- Engage local general practice, clinicians, consumers, Maori Health, and Service Managers in the project leadership, development and agreement of the new integrated model
- Make an assessment of the costs and benefits of this model of care including the operational costs of implementation of the new model, impact on equity, benefits realisation
- Consider options for policy, contracting, funding and governance arrangements in a new model across the whole health system
